Welcome to our Biological Dental Practice.
At the present time we are experiencing a great deal of interest in biological dentistry and the role dental and general oral health conditions affect overall health. We encourage anyone interested in joining our practice to review our website and to determine if our practice philosophy and approach meets your needs. If so, we ask that you contact the office by phone and speak with either Annie or Sue for information on how to proceed. In general we will need the following information:
1. A current digital panoramic x-ray, like the example below. We may request any xrays you need have had in the last 12 months.

2. A completed medical history questionnaire, which will be forwarded to you.
3. Current vitamin D3 blood levels. If you have other blood work done recently we would like to have those results also. Vitamin D levels are mandatory. The reason we require this metric is that it impacts much of what we do in dentistry.
